We are seeking a skilled and compassionate Physical Therapist to join our team for a home health position. As a Physical Therapist in home health, you will be responsible for providing comprehensive physical therapy services to patients in their homes. Your duties will include assessing patient needs, developing and implementing treatment plans, monitoring progress, and communicating with the patient's healthcare team.
- A. Conduct patient evaluations and develop individualized treatment plans
- B. Provide hands-on physical therapy interventions and exercises
- C. Monitor and document patient progress and adjust treatment plans as needed
- D. Communicate with patients, families, and other healthcare professionals to coordinate care
- E. Provide education to patients and families regarding their physical therapy treatment
- F. Maintain accurate and up-to-date patient records
- G. Follow all state and federal regulations and adhere to ethical standards
Home Health Experience Required
CA License Required
OASIS EPIC experience preferred
