*Provisional Appointments: For a provisional appointment, a civil service exam is not required. However, to obtain a regular position, the appointee will need to compete successfully in a County Exam when open.

The PROGRAM 

Alameda County Health Care for the Homeless (ACHCH) strives to eliminate barriers to health care and increase housing opportunities for people experiencing homelessness. ACHCH provides wrap-around services, outreach and linkage to support access to medical, behavioral health, dental, and optometry services for people who are unhoused. ACHCH has FQHC designation and delivers services through a combined approach of ACHCH providers and staff who serve patients directly, and through ACHCH-contracted providers in the area (public hospital system, FQHCs).

In recent years, with the number of unhoused reaching more than 9,000 people, we rapidly increased street health outreach services and shelter-based health services throughout Alameda County. We have been on the front line of responding to the COVID pandemic for the homeless community since March 2020.

Major Tasks, Duties and Responsibilities:

  • Provides education and support for HCH staff, programs, clients, and local Alliance of the Mentally Ill groups.
  • Acts as a liaison in providing pharmacy system support and interface with pharmacy benefit management (PBM) and electronic prescribing systems, physicians, and pharmacies.
  • Provides oversight and monitors of the procurement, storage, usage, and disposal of program supply of pharmaceuticals and related medical supplies.
  • Acts as a liaison with drug company medication and Patient Assistance Programs (PAP) by generation of medication reports and patient assistance applications.
  • Supports and maintains development and preventative health and harm reduction interventions or services across the program including immunizations.
  • Participates in the development of program quality improvement projects and quality assurance committees.
  • Compiles and maintains accurate records and files regarding psychotropic medications, medications for substance use disorders, and prepares periodic and special statistical or narrative reports as assigned by the Pharmacy Director.
  • Participates in ongoing Continuing Medical Education (CME) for all HCH staff.
  • Coordinates the Adverse Drug Reaction reporting system and medication safety committees.
  • Provides psychopharmacologic and substance use disorder consultations and advisory as requested by the HCH medical staff and their contractors.
  • Keeps informed of new drugs and innovations in the pharmaceutical field, maintains awareness of current literature and information for HCH staff, serves as a resource for the professional staff and provides technical assistance for HCH staff and their contractors.
  • Perform other duties as assigned within the classification.
Job Requirements: 

SENIOR CLINICAL PHARMACIST

MINIMUM QUALIFICATIONS

LICENSE:  Possession of a valid California Registered Pharmacist. – AND –

EXPERIENCE:  Two (2) years of professional level experience as a pharmacist working with mentally ill clients. -OR-

EXPERIENCE:  Completion of a pharmacy residency or a fellowship program (psychiatric training program preferred). -OR-

EDUCATION:  Possession of a Board of Pharmacy Specialties Certification; Board Certified Psychiatric Pharmacist (BCPP) is preferred.
