Indira Care home Health is looking to add a Full-Time/Part-Time Physical Therapist to care for our outpatient.

Responsibilities includes but not limited to:

1. Conduct comnprehensive Physical therapy evaluation in Patients' Homes.
2. Develop and implement indivisualuzed treatment plans based on patients needs, goal and Physician order.
3. Monitor and document patient progress, adjust treatment plans as necessary and communicate updates to interdisciplinary team
4. Educate patiemts, caregivers, and family member on excersise, safty practice
