We are currently seeking a Pediatric Social Worker to provide and coordinate home services to pediatric hospice and pediatric palliative care cases in the East Bay/Alameda area.  This person will be responsible for providing supportive services to help meet patient/family social, emotional and financial needs related to the impact of a terminal illness, loss and grief, in compliance with organization policies and procedures and applicable laws and regulations. Essential Duties & Responsibilities (include but are not limited to):

• Assess patient/family social and emotional factors to determine ability to cope with terminal illness and death.
• Assess family/caregiver capacity to participate in providing care to patient.
• Provide individual and family counseling and support services to patient/families in all categories of hospice care.
• Assess for potential risk for suicide and/or abuse or neglect.
• Assess environmental resources and obstacles to maintaining safety.
• Assess special needs related to cultural diversity including communication, role of family members and special traditions.
• Establish treatment goals and revise counseling interventions as indicated. Evaluate patient/family response to psychosocial interventions; identify obstacles to compliance and assist in understanding goals of intervention.
