What You'll Do:

As a LVN, you’ll be responsible for providing care for clients in their homes. Your duties will include:

  • Conducting comprehensive assessments to determine clients' needs.
  • Carrying out personalized care plans.
  • Coordinating with physicians, therapists, and other healthcare professionals to ensure seamless care delivery.
  • Monitoring and adjusting care plans as needed to meet clients' evolving needs.
  • Providing education and support to clients and their families.

Job Title/Position: Licensed Practical/Vocational Nurse

Reports To: Clinical Manager / Director of Patient Care Services Job Description Summary The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Essential Job Functions/Responsibilities 

  • Provides direct patient care as defined in the state Nurse Practice Act.
  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
  • Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the registered nurse.

Job Requirements: 

Position Qualifications 

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Has at least one (1) year nursing experience. Community/home health or medical/surgical experience is preferred.
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.

Job Title/Position: Licensed Practical/Vocational Nurse

  • Possess and maintain valid CPR certification.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Environmental Conditions: May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying degrees of repair.

Working Conditions: May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances. May be exposed or occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required. OSHA exposure category: Category I — Position includes tasks that involve exposure to blood, body fluids, tissues.

 

The service areas for this position are San Francisco, San Mateo, and Santa Clara counties.
