POSITION TITLE: Licensed Clinical Social Worker (LCSW)
DEPARTMENT: Behavioral Health
REPORTS TO: Behavioral Health Manager
SUPERVISION GIVEN: Individual and group
JOB SUMMARY: The LCSW I is responsible for providing quality behavioral healthcare services to patients including psychosocial assessment, intervention, referral and consultation to providers. This position is a combination of remote and on-site work.
KNOWLEDGE OF WORK
Knowledge of the current Diagnostic Statistical Manual of Mental Disorders and ability to accurately use this knowledge to diagnose patients and inform treatment planning
Knowledge of psychotropic medications and physical health conditions/chronic disease
Knowledge of crisis intervention, applicable laws and ethical consideration related to patient rights, treatment, safety and confidentiality
Knowledge of Microsoft Office required
Knowledge of Federally Qualified Health Centers and the key health center principles and guidelines
DUTIES AND RESPONSIBILITIES
Prioritize, schedule and conduct brief evidence-based therapy for patients with mild to moderate behavioral health symptoms and health behavior issues
Assess, refer and advocate when appropriate, for services outside of the brief therapy model of care
Provide crisis intervention as needed
Provide education and support to patients and their families to encourage compliance with medical and behavioral regimes
Communicate and collaborate with appropriate community partners
Provide consultation to appropriate primary care staff and providers on behavioral health issues of patients and guide and assist in making CPS/APS reports or contacting law enforcement as needed
Document patient visits and related information into the electronic medical record in a timely and thorough mannerDevelop and present trainings, in-services, and updates to providers and staff on behavioral health topics
Write letters documenting and/or summarizing diagnosis and treatment as requested and appropriate
Attend and present at group and individual supervision, in a frequency prescribed by the Behavioral Health SupervisorPerform other duties as assigned
EDUCATION, EXPERIENCE, TRAINING
- Masters in Social Work from an accredited school of social work required.
- Current California Licensed Clinical Social Worker license required.
- Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred.
- Strong interpersonal and customer service skills required; ability to handle difficult patients.
- Strong computer skills; electronic health record system preferred.
- Must have strong analytical and problem-solving skills.
- Must certify and remain current in CPR.
