The Street Health Team is a dynamic multi-disciplinary team made up of Behavioral Health Clinicians, Nurses, Health Workers, peers, physicians, and psychiatrists. Teams work in assigned neighborhoods in San Francisco to engage and address the complex health and social needs of people experiencing homelessness. The teams collaborate closely with other city departments, including the Department of Homelessness and Supportive Housing, to help people come indoors and receive treatment and others supports. This is a great opportunity for candidates who want to be part of helping some of San Francisco’s most vulnerable populations and participating in the City’s developing multi-pronged strategy to reduce homelessness and help people achieve long-term stability and health.
- The Behavioral Health Clinician is an integral part of the team, working with a caseload of clients with behavioral health needs, performing clinical assessments, care planning, and other trauma informed interventions to meet client goals.
